Care Finder expansion strengthens support for older Territorians

Anglicare NT is proud to announce the expansion of the Care Finder service in the Darwin urban area, made possible through additional funding from the Northern Territory Primary Health Network (NT PHN). This investment strengthens our ongoing commitment to ensuring older Territorians can access the aged care supports they need to live with dignity, safety and choice.

Anglicare NT has delivered the Care Finder service across the Northern Territory since 2023, connecting hundreds of older people with essential aged care supports. The free service is designed to support older people who are vulnerable, isolated or experiencing homelessness by providing a personalised and compassionate pathway into aged care.

Care Finder specialists assist older people to understand available aged care options, organise assessments and navigate services with confidence. Through advocacy, practical support and collaboration with local providers, Care Finders ensure individuals receive the right services at the right time. They also review options as needs change, ensuring ongoing, responsive support.

For older people experiencing homelessness, Care Finder provides critical assistance. We help people to identify interim accommodation and transition smoothly into safer living arrangements with aged care services in place.

“This expansion means more Territorians will be supported to live the lives they choose, in a way that honours their independence and wellbeing,” said Care Finder Coordinator Vanessa Johnson.

“Care Finder is a fantastic service. Our team works alongside older people to get the supports they deserve and are entitled to, helping them feel safe and respected. Not everyone has easy access to a mobile phone, computer or printer. We work with them to set up services to make their life easier while maintaining their independence.”

“Some people apologise for the assistance we give them. But it’s what people deserve. It’s what I would do for my mum,” said Vanessa.
