HOME AND COMMUNITY CARE Transport, Respite and Social Support

The HACC Transport service is for frail aged people 65 years and over and younger people with a disability, and their partners or carers who live in the Darwin , Palmerston and Rural areas. The service provides:

• Transport, support, advocacy, information, counselling and referral for individuals.
• The Wednesday Leisure Group is a supported social group for those with mild to moderate memory loss, providing respite for carers.
• The Friday Men’s Group is a supported social group specifically for men who experience social isolation and may have mild to moderate memory loss and/or an acquired brain injury.
